Located in the Caldwell Medical Arts Building, our state-of-the-art office in Caldwell shares a parking lot with the West Valley Medical Center, making it easy to find and convenient for patients from Nampa, Middleton, Parma, and other nearby communities. We’re open for appointments from 8 a.m. to 5 p.m., Monday through Thursday, and from 8 a.m. to 3 p.m. on Friday. The building and our office are wheelchair-accessible and free parking is available.

  • Head northwest on 1st St S toward 11th Ave S

  • Follow I-84 W to US-20 E/US-26 E/Franklin Rd in Caldwell. Take exit 29 from I-84 W
